Feedback, Complaint & Appeal form

All student forms must be lodged online through the iStudi Student Portal.
Paper or email submissions will not be accepted.

How to Access the Forms

1. Log in to iStudi (Student Portal) using your student ID and password.
2. Go to Requests → Student Forms.
3. Select the form you need from the list above.
4. Complete all required fields and upload supporting documents (if applicable).
5. Submit the form. You will receive a confirmation email and a ticket number.
6. Track your submission under My Requests in iStudi.
